When you schedule an appointment to establish care with one of our providers, you will need to complete some necessary paperwork. Listed below are the forms which will be needed to start your chart at our clinic:
Patient Registration Information tells us about you, your insurance information, and your contact information.
This notice describes how medical information about you may be used and disclosed and how you can get access to this information.
HIPPA Authorization form tells us how you authorize us to use and to whom you allow us to release your private health information.
Medical Records Release Form authorizes us to obtain prior medical records from your former physician.
Pediatric Questionnaire should be completed prior to the first visit when the patient is age 15 or younger.
